when did this happen? and how do you damage someone else by reviving? is this a bug?

It's Amumu's sunflare cloak. That's a known bug.
Somehow I once was considered as assisting in my own death, dealing ~50% true damage to myself. I was playing Syndra.
I didn't get an assist for it )':
Never trust that thing. lol.
The death recap has been broken for years but Riot just completely ignores it. Almost nothing is accurate and is just downright comical sometimes.
