Champ Select Broke

I invited my friend at the same exact time I entered matchmaking (i think?) and he said he joined but it didn't work, I cancelled matchmaking and checked pending, it said he was in but he wasnt. He got this when I tried to invite him: http://prntscr.com/9ryexp

and then I had him invite me, it did nothing after I pressed accept.

I made a lobby and it said {0} is already a member of a group.
