[Client]CancelQueue with bad timing
If you cancel queue at the same time the client find a match and you appcept. You are considered in champ selection at the same time you're in the menu doing something else like editing runes, forming a group or even in queue. I was duoing with a friend, he said I was in champion selection but afk. The inactivity made me miss the champ select.
